Low-Cost Spay & Neuter Registration
Please note that this form should be used to register any dogs (regardless of your county) as well as cats that live outside of the five counties (Montgomery, Rowan, Bath, Menifee and Morgan) which are covered by our Community Cats Spay & Neuter Program.

Please complete one form for each pet you wish to get on the schedule. Registrations with multiple pets on one form will be deleted.
If there is space available in one of our clinics, you will be notified after registration. Please allow several business days for your registration to be manually processed by our volunteer team.
Payment must be made to secure your appointment. If you fail to pay before the clinic fills up, you will not be put on the schedule.
PRICING
SPAY/NEUTER FEES:
Male Cat: $79 + tax
Female Cat: $89 + tax **
Male Dog: $125 + tax
Female Dog: $150 + tax **
We are not currently able to see Female dogs over 75 pounds.
**If your female pet is in heat or pregnant, you may be charged a $50 additional fee depending on complications arising from these situations.
**If your pet has a Pyometra (A serious and potentially life-threatening infection of the uterus that causes it to fill with bacteria and pus in un-spayed females), you may be charged an additional $50 due to complications and additional surgery time due to the seriousness of the ailment plus the cost of antibiotics. (generally around $500-$1500 when taken to an emergency vet).
**If your pet is deemed too risky for surgery, the vet may opt not to perform the requested services. In this event, you will be refunded partially depending on what services were performed (anesthesia, vaccines, microchipping). You will be fully refunded if your pet doesn’t receive any services. This is an attempt to protect your pet’s well-being as we strive to do no harm.
**Price includes annual vaccinations, rabies, pain relief injection, ear cleaning, & nail trimming. Rabies is required for all dogs and cats entering the clinic unless proof of current vaccine is presented.
**Depending upon the vet, prices may vary. The pricing listed in the marketing on FaceBook or the graphic for the specific clinic will show the price of the subject clinic.
**6% Kentucky state sales tax is added to all services.
**After you register, you will be invoiced for the amount owed. Your pet will not be seen if you do not pay your invoice before the clinic fills up. Clinics fill up fast. Please pay upon receipt.
